Hold Harmless Agreement

I understand that participation in Vestibular Rehabilitation Therapy on 9/13/25 and 9/14/25 at Touro may involve physical activity and carries a risk of injury.

By participating, I agree to release and hold harmless Advanced Neuro Therapy Education and Moreau Physical Therapy, its instructors, and affiliates from any liability for injury, loss, or damage arising from my participation, whether caused by negligence or otherwise. Participation in labs can be modified as needed. Please let instructor know in the event of illness, pregnancy, or any other condition so that adjustments can be made.

I affirm that I am physically able to participate and assume all associated risks.
